Students needing to ask for an exception to their assigned Commencement date and time can request that here.
Academic advisors may initiate competency status override requests as warranted by professional judgment.
This request does not remove the missing competency status. The Registrar’s Office will support the student and register them for the courses listed below.
Department Notification - Student Initiated Drop/Cancellation
Name Changes, SSN Changes, and Date of Birth Corrections for the Registrar's Office
Students are permitted to offset equal credits and course fees with the recommendation of academic advisors and academic department chairs.
This must happen between the 10th and 25th calendar day after the course begins.
Verification of Non-Participation
This form cannot be submitted until the student is enrolled in courses and the new major has been declared.
This is used to change the campus from main campus to online or vice versa.
This form is not used to withdraw from a currently enrolled course.
Use this form to request a reinstatement back into a course that was withdrawn from in error.
Use this form to authorize the release of information under FERPA.
